User Guide: Leave Application Excel Template

1. How to Download & Open the File

  • Click on the generated file link above (Leave_Application_Templates_10_Formats.xlsx) to download it to your computer or device.
  • Open the file in Microsoft Excel, Google Sheets, or LibreOffice Calc.

2. Navigating the Workbook

Directory Index Sheet (Index & Guide):

  • Open this sheet first to view the master list of all 10 application types.
  • Click any blue "Go to [Sheet_Name]" hyperlink to jump directly to that specific form.

Returning to Index:

  • On every individual leave sheet, click the ← Back to Directory Index link in cell B2 to return to the main dashboard.

3. Customizing for Your Organization (One-Time Setup)

Before sending or printing forms, update the template defaults to match your company details:

  • Company Header: Update COMPANY / ORGANIZATION NAME in row 6 to your company or department name.
  • Employee Details: Replace the sample placeholder text (e.g., John Doe, EMP-1024, Operations/IT) with your actual details.

4. How to Fill Out a Leave Request

A. Entering Dates & Auto-Calculation

  • Input your Start Date (From) in cell C12 (format: YYYY-MM-DD or DD/MM/YYYY).
  • Input your End Date (To) in cell E12.
  • The Total Leave Days cell automatically calculates the exact number of days using the formula =End_Date - Start_Date + 1.

B. Filling Out Category-Specific Fields

Depending on the leave format you choose, fill in the custom details in Section 2:

  • Medical Leave: Add your doctor/hospital name and indicate if a medical certificate is attached.
  • Half-Day Leave: Specify whether it is for the First Half or Second Half and set the time range.
  • Study / Exam Leave: Enter the examination title, date, and institution.
  • Annual Leave: Mention your remaining paid leave balance and emergency contact details.

C. Reason & Work Handover

  • Type a concise explanation in the Reason for Leave box.
  • Enter the name of the colleague covering your duties in the Work Handover To field.

5. Printing or Saving as PDF

  • To Print: Select the desired leave sheet, press Ctrl + P (or Cmd + P on Mac). The printable area and gridlines are pre-formatted for clean single-page layout.
  • To Export as PDF: Go to File > Save As / Export > PDF to send a digital copy to your Manager or HR department for approval.
