Here is a basic template for a wedding guest list:

1. Name of guest

2. Relationship to you (e.g. cousin, friend, coworker)

3. Address

4. Phone number

5. Email address

6. Plus one (if applicable)

You may also want to include columns for RSVP status and any dietary restrictions or special accommodations that the guest may have.
